General improvements
REQ-203: Adds a warning message which displays when a user's extension is already in use and someone logs in using the same extension.
NOTE: Requires TCC 5.3.0 and App 5.3.0
SUS-1420: Fixes an issue which caused the app to intermittently fail to load dispositions associated with a matched record, unless the app was triggered to reload dispositions (by saving notes, or otherwise causing the app to reload).

Known Issues
TF-13667: When contextual matching is configured for SFDC and Dynamics object types, the matched record does not always display at the bottom of the UI.
